COVID-19 Self Screening

If you have NOT been previously diagnosed with COVID-19:
  1.  (YES/NO) Do you currently have, or have you experienced any of the following symptoms in the past 14 days?
            • fever
            • cough
            • shortness of breath or difficulty breathing
            • chills or shaking with chills
            • new loss of taste and/or smell
            • muscle pain/aches
            • fatigue
            • sore throat
            • GI upset or diarrhea
  2. (YES/NO) In the past 14 days, have you been in close contact with anyone who has exhibited these symptoms, or who has been confirmed to be COVID-19 positive?
  3. (YES/NO) In the past 14 days, have you traveled to abroad, or to any regions or “hot spots” affected by COVID-19?
  4. (YES/NO) Do you currently have any of the following symptoms?
            • trouble breathing
            • persistent pain or pressure in the chest
            • new confusion
            • inability to wake or stay awake
            • blueish lips or face
  •  If you answered “yes” to any of the questions above, please call the shop at (206)365-8282 to have your appointment rescheduled.  Please consider the health and safety of the artists, employees, and other clients, who may be at higher risk for severe illness.  Your deposit will NOT be affected, and will still be valid for your new appointment time even if you call on the day of your appointment.  If you do not call and do not show up to your appointment, we will assume you no longer want the tattoo, and your deposit will be forfeit.  If you have any of the symptoms in question 4, the CDC suggests you seek immediate emergency medical care.
  •  If you are at higher risk for severe illness for any reason (more on that HERE) and don’t yet feel comfortable being in close contact with others, you are welcome to call the shop at (206)365-8282 and have your appointment rescheduled or put on hold.  Your deposit will NOT be affected, and will stay valid until you reschedule, even if you call on the day of your appointment.  If you do not call and do not show up to your appointment, we will assume you no longer want the tattoo, and your deposit will be forfeit.  Deposits are not refundable.
If you’ve had COVID-19, or have home isolated with symptoms:
  • Please consult your doctor, healthcare provider, or local health department to determine when it is safe for you, given your specific circumstances, to be in public and, specifically, in close contact with another person as you would be while getting tattooed.  The CDC offers guidance on the discontinuation of isolation (HERE and HERE); but, as stated by the CDC, follow the direction of your doctor or local health department.  As new information becomes available, what you have heard or read in the past, may have changed.  Again, please consider the health and safety of the artists, employees, and other clients, who may be at higher risk for severe illness.  If you need to reschedule, please call the shop at (206)365-8282.  Your deposit will NOT be affected, and will stay valid for your new appointment time even if you call on the day of your appointment.  If you do not call and do not show up to your appointment, we will assume you no longer want the tattoo, and your deposit will be forfeit.
